Salary Range: $57,000-$60,000/YR | Location: Mount Prospect, IL
Schedule: Monday to Friday, 1:00 PM to 9:00 PM, with a rotating on-call schedule.

About the Role: As a Supported Living Program Coordinator, you will oversee several SLP program sites, coordinating services and supports for individuals. You will ensure daily staffing needs are met, maintain high-quality standards at all sites, and work closely with Adult Learning, Medical, and Behavioral Health services. The position also includes on-call responsibilities and reports to an SLP Program Manager.
Job Responsibilities:
Administrative/Professional:
- Champion the Mission Statement and agency values of Search.

- Represent Search professionally to family members, guardians, community members, and affiliated agencies.
- Manage your workload effectively and meet established deadlines.
- Serve as a role model and mentor to staff, fostering the development of their skills and competencies.
- Create and maintain staffing schedules for all sites, covering shifts as needed.
- Provide on-call coverage on a rotating basis.
- Ensure program staff are fully trained in all required topics.
- Collaborate to ensure timely completion of incident reports.
- Manage supply purchasing, Link cards, petty cash, credit card charges, DASH monies, and individual trust funds efficiently.
- Ensure accurate daily documentation and recordkeeping on individual progress and other tracking documents.
- Adhere to procedures related to medication administration, diet training, compliance with physician orders, and other required trainings.
- Maintain compliance with personnel policies as stated in the employee handbook.
- Understand and comply with Corporate Compliance policies, employee work rules, and state/federal regulations related to Search services, such as DHS Rules and CQL standards.
- Follow vehicle safety procedures and complete vehicle maintenance reports as requested.
- Complete other duties as assigned by your supervisor.
Individual Interaction:
- Demonstrate kindness, courtesy, flexibility, responsiveness, and consistency in meeting individual and program needs.
- Ensure individual medical needs are met compassionately and efficiently.
- Ensure program staff effectively assist individuals with daily living activities and personal self-care.
- Ensure individuals are planning, participating, and engaging in community activities, including transportation and assistance.
- Ensure staffing is available at program sites and community activities for safe execution.
- Provide verbal de-escalation strategies when needed, including during unscheduled work time.
- Assist with medical appointments and emergency medical events as requested.
Cleanliness and Order:
- Ensure program staff maintain the physical environment, grounds, and vehicles according to Search standards for cleanliness and order.
- Ensure timely completion of QA Plans of Correction and staff awareness of necessary corrections.
- Collaborate with Physical Plant staff to ensure program sites meet maintenance and upkeep standards, including furnishings, appliances, and landscaping.
Minimum Requirements:
- High school diploma or equivalent.
- Ability to lift/carry up to 44 lbs. and perform basic physical moves such as kneeling, reaching, crouching, bending, and twisting.
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
- Valid driver’s license, reliable vehicle, and proof of vehicle insurance.
- Maintain Pace certification and good standing on the Illinois Healthcare Worker Registry, including CPR and First Aid requirements.
Preferred Experience:
- Bachelor’s degree in psychology, Social Work, or related field.
- At least 3 years of Direct Support Professional and/or management experience.
- Familiarity with applicable regulations and standards, including DHS Rule 115 and CQL standards.
