General Summary
The Case Manager Supervisor oversees a team of case managers, ensuring that they provide effective and efficient service to clients in need of guidance, support, and resources. This role involves coordinating the team's efforts, monitoring the quality of care and service provided, and implementing strategies to meet organizational goals. By fostering a collaborative environment, the supervisor ensures that case managers are well-equipped to address the diverse needs of their clients. Through leadership and oversight, the Case Manager Supervisor plays an integral role in ensuring that clients receive the support they need to navigate their challenges, while also contributing to the continuous improvement of the organization's service standards.
Essential Duties and Responsibilities
Conducts weekly supervisions with Case Management Personnel at the direction of the Program/District Manager to ensure deliverables.
Monitors service delivery to individual clients to ensure services are being provided as appropriate and effectively meets the client's needs.
Delegates/Conducts mandated State Trainings during supervisions as needed.
Thoroughly understand the scope and purpose of the State Regulations.
Conducts or arranges in-person training and oversight of production for new/existing Case Management personnel.
Keeps Program/District Manager informed of progress and problems associated with duties.
Reports staff performance issues to Program/District Manager.
Motivate Case Management Personnel for quality service delivery and daily expectations.
Assures that statistical data is accurate, compiled and submitted on a regular basis in the State Database and Care Lync internal data tracking.
Meets with Program/District Manager at least weekly to discuss and review cases and Case Manager performances.
Submits PIP to Program/District Manager for approval.
Meets with Case Manager to deliver and monitor PIP.
Assists in development and revision of workflow and procedures as needed.
Attends organizational meetings, and training as required.
Adhere to high ethical standards as well as Care-Lync agency standards.
All other duties assigned.
Qualifications
Bachelor's degree in social work, sociology, psychology, or a related field, AND
Three years' experience in the human service or health related field.
Ability to effectively coordinate and communicate with clients, service providers, public, and other staff members.
Ability to motivate staff members.
Skills in team building and group dynamics.
Knowledge of community organization and service system development
Problem solving skills and techniques.
