Program Description
Serving adults and transitional age youth with serious mental illness, the Adult Full Service Partnership (FSP), TAY FSP, TAY Acute Linkage, and Adult Intensive Case Management programs provide an extensive array of recovery-oriented, age-appropriate services and supports, including linkage to housing and basic needs assistance, physical health care, benefits assistance, employment services, and integrated mental health and substance abuse treatment services. The programs follow an assertive community treatment model, providing medication and other wraparound services with case management for those with chronic serious mental illness. This continuum of services provides outpatient mental health treatment to 18-60+ year-olds living with severe mental illness. The programs consist of multi-disciplinary teams consisting of a psychiatrist, a psychiatric nurse practitioner, licensed vocational nurses, clinical program directors, clinical case managers, peer outreach and support specialists, and an administrative manager. Services are provided to clients both in the office and in the field, as needed.
Job Description
Under the direction of a licensed psychiatrist, exercises medical responsibilities for psychiatric evaluation, medication assessment and medication monitoring. Supervises two licensed vocational nurses. Performs crisis intervention, supportive counseling, and other direct mental health services, including case management, and assists with 5150 evaluations, as needed. Orders diagnostic tests, communicates medical issues with Primary Health Care providers. Maintains client caseload and clinical records in accordance with mandated standards. Directs treatment planning and works as a member of multi-disciplinary team. Consults and provides on-going education to non-medical staff in psychopharmacology and medical aspects of patient care. Services will be provided either at Felton itself, in patients’ homes, or in other field locations as needed. Additional duties as needed.
Program Specific Responsibilities
- Exercises medical responsibilities for psychiatric evaluation, medication assessment and medication monitoring
- Orders diagnostic tests as needed
- Coordinate medical issues with Primary Health Care providers
- Maintains client caseload, as needed
- Maintains clinical records in accordance with mandated standards
- Supervises the Licensed Vocational Nurses (LVN)
- Management of timecards
- Direct staff's daily and weekly tasks
- Meet weekly, or as needed, with LVNs, to discuss clinical, operational, managerial, client, and/or medication issues, questions, or concerns
- Provide consultation with LVNs as needed
- Help team prioritize tasks for maintaining compliance of medication room
- Facilitate meetings with medical team as needed
- Support with hiring and training of staff
- Consults and provides on-going education to non-medical staff in psychopharmacology and medical aspects of patient care
- Consults regularly with on-site physician on new cases and medication adjustments according to your agreement
Minimum Qualifications:
- A minimum of 3 years of Psychiatric Nursing experience prescribing and treating mental health patients preferred
- Verifiable completion of training in an accredited Nurse Practitioner Program
- Current Nurse Practitioner license
- Current furnishing number license from State of California
- Current DEA registration
- Demonstrated commitment to the welfare of individuals and families with mental/behavioral health issues
- Prior supervisory experience desired but not required
- Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance
- Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and psychosis. This includes symptoms, treatment alternatives, drug properties and interactions, and preventative healthcare measures
- Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders
- Complex problem solving; identifying complex problems and reviewing related information to develop and evaluate options and implement solutions
- Works well with a multidisciplinary team- able to receive feedback and collaborate well with peers and supervisors
- Demonstrated ability to work sensitively with diverse people and communities
