Speech Language Pathologist, SLP Job Description:
The Speech-Language Pathologist (SLP) is responsible for improving residents' communication and swallowing abilities.
Speech Language Pathologist, SLP Responsibilities:
Assessment and Evaluation: Conduct thorough assessments of residents' speech, language, cognitive, and swallowing abilities.
Individualized Care Plans: Develop customized intervention plans to address communication and swallowing difficulties.
Section GG Data Collection: Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, and Significant Change assessments, as applicable.
Treatment: Administer speech therapy interventions, including exercises, communication strategies, and swallowing therapy.
Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with Point of Service (POS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).
Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
Interdisciplinary Collaboration: Collaborate with other rehab staff, such as Physical Therapists and Occupational Therapists, to provide holistic care for residents.
Speech Language Pathologist, SLP Qualifications:
State licensure as a Speech-Language Pathologist.
CPR/BLS certification.
