- Supervised Post-Certification in an institutional setting (1+ years)
- 1+ years of experience in any of the following:
- Home Health Care
- Epic Home Health
- Home Health
- Valid SLP license in the state of Illinois (IL)
- Any of the following valid licenses/certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- American Heart Association Pediatric Advanced Life Support (AHA PALS)
Preferred

Overview
The Speech Therapist (Pathologist) will provide speech therapy services to Agency patients as prescribed by the attending physician, offering consultation to Agency staff and the patient’s physician as needed. Responsibilities include assessing, planning, and implementing rehabilitation services for speech, language, and/or swallowing disorders, evaluating speech and language abilities through diagnostic testing, and recording/reporting the client’s progress and any changes in condition. The therapist will also instruct family members and healthcare team members on assisting the patient in improving communication skills and maintain clinical and progress notes.
Key Responsibilities
- Assess, plan, and implement rehabilitation services for speech, language, and/or swallowing disorders.
- Conduct diagnostic evaluations of speech and language abilities, establish goals, and estimate recovery prognosis.
- Document the client’s progress and communicate any changes in the patient’s condition to the physician.
- Instruct family members and healthcare team members on methods to assist the patient in improving communication skills.
- Re-evaluate patient progress and adjust the rehabilitation program as necessary.
- Prepare periodic reports for the Agency, physician, and third-party payers regarding care.
- Assist with referrals to external audiology or speech clinics and other community services as indicated.
- Maintain adequate clinical and progress notes, submitting required paperwork in a timely manner according to Agency policy.
- Serve as a liaison and provide follow-up with community agencies.
- Participate in in-service programs, case conferences, and staff meetings as required.
- Maintain and update professional competency through in-service attendance and professional resources.
- Perform additional duties as assigned by the supervisor.
Schedule and Shift Details
- Schedule: Weekdays, day shift; no weekends, on-call, or holidays
- Hours Needed: 20 hours per week, 3 days per week
Travel Requirements
- Employee will travel by automobile and be exposed to changing weather conditions.
Position Title: Home Health SLP, Part-Time — $10,000 Bonus (JR243049)
- Compensation: PPV + $5k FYS bonus + $5k sign-on bonus
- Hours Needed: 20 hours per week, 3 days per week
- Schedule: Weekdays, day shift; no weekends, on-call, or holidays
- Coverage Area: Addison, Glendale Heights, Carol Stream, Elgin, Bloomingdale, Roselle, Hanover Park, Bartlett, Itasca, Elmhurst, Hillside, Villa Park, Bensenville, & surrounding areas
- Requirements: IL SLP license, Master’s degree, minimum 1 year of healthcare experience, geriatric care experience (e.g., AL, LTC, NH). Ideal experience includes Home Health (HH).
