Provides skilled speech pathology service on an intermittent basis to patients in their homes in accordance with the physician's orders.
POSITION REQUIREMENTS - JOB KNOWLEDGE, PERSONAL ABILITIES:
- Assesses and evaluates the nutritional status of patients.
- Assists the physician and nursing personnel to determine the dietary needs of the patient.
- Prepares and calculates therapeutic diets to meet specific requirements of the patient and confers with the physician to establish dietary treatment.
- Assists the patient and family to understand and follow dietary modifications ordered by the physician. Instructs on dietary restrictions and drug/nutrient interactions as appropriate.
- Observes, and reports to the physician and nurse supervisor the patient's response to dietary treatment and any related changes in the patient's condition and revises nutritional plan as indicated by patient's condition.
- Participates in care coordination activities and acts as a resource to other health care staff, as requested.
- Participates in Quality Improvement activities as assigned and communicates information that enables collection and analysis of data to identify opportunities for improvement.
- Participates in the development of periodic revision of the physician's plan of treatment keeping him/her informed of the patient's status and obtaining additional orders if necessary.
- Communicates with other team members and, when appropriate, instructs them in nutritional, dietary teaching that they may use while working with the patient.
- Maintains and submits written clinical and agency records as deemed necessary.
- Participates with staff, patient, and physician in discharge activities and completes a Dietary Discharge Summary.
- Assumes responsibility for self-development by continually striving to improve his/her health care knowledge through educational programs, attendance at workshops and conferences, active participation in professional and related organization meetings and individual research and reading.
- Confirms, on a weekly basis, the scheduling of visits with the Director of Nurses to coordinate Agency visits.
- Performs additional duties assigned in response to patient and/or Agency needs.
- Obtains a medical history, particularly as it relates to the present condition.
- Evaluates the patient, the patient's significant other(s) and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the patient, and what other Agency and community services will be required.
- Assures that the physician's orders are appropriate and discusses necessary changes.
QUALIFICATION
- The ability to drive and the ability to access patients' homes which may not be routinely wheelchair accessible are required.
- Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.
- On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds.
- The ability to speak, write & read in English.

BENEFITS
- Salary range $38,400.00 - $85,000.00 (based on several factors including but not limited to education, work experience, certifications, etc.)
- Office hours 9:00AM to 5:00PM
