Evolve Therapy Services is an industry leading therapy management organization providing seamless therapy recruitment for our long-term care industry clients. We are looking for compassionate Speech Language Pathologist, to give quality care for one of our AMAZING affiliated SNF facilities, Champion Care located in Kenosha, WI.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility *Must have active WI state Speech Language Pathologist license

Job Type: Full Time includes $7,500 SIGN ON BONUS!

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions could include:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards. 
• To evaluate and obtain data necessary for treatment planning and implementation 
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities 
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• Current State license (or is license-eligible) in state of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
