Summary

While performing duties related to St. Clair Hospital, this position is responsible for the provision clinical pharmacy services in support of organizational goals and objectives. This is a heart failure pharmacist specialty position that will work in support of the inpatient heart failure service. Works to support and facilitate optional patient outcomes through safe, cost-effective drug therapies. Works with the Director of Pharmacy to ensure compliance with laws, regulations and acceptable standards of practice. May perform duties of a staff pharmacist.

Minimum Qualifications

Graduate of an accredited school of pharmacy.

Completion of an accredited post-graduate year 1 (PGY1) residency or 3 years of comparable health system pharmacy experience.

Pennsylvania licensed pharmacist or eligible for licensure within 60 days of hire.

Preferred Qualifications

Completion of an accredited hospital-related post-graduate year 2 (PGY2) residency.

Board of Pharmacy Specialties certified.

Two years of experience in a health system pharmacy clinical position.
