Job DescriptionThe Special Procedure Patient Navigator - Medical Assistant will perform the duties necessary to coordinate the care of the patients who undergo special procedures such as a colonoscopy, esophagogastroduodenoscopy (EGD), or Stress Test as part of the Special Procedures Program at Salud Family Health. The Special Procedure Patient Navigator approaches his/her tasks in a team-based fashion that supports patients and their families in self-management, self-efficacy, and behavior change.

- Starting wage range is $20.50 to $27 per hr.
- 5-day work week – Monday to Friday – Available 8 am to 5 pm

Supervision Received:
Works under the clinical supervision of the Site Proceduralist and the administrative supervision of the Vice President of Operations or their designate
Primary Duties:
- Provides Patient Navigation services to assist patients with appointment scheduling, preparation, and follow-up for special procedures such as stress tests, EGDs, or colorectal cancer screening
- Determines patient eligibility in special programs and implements all Colorado Colorectal Cancer Screening Program (CCSP) requirements, attends programs calls and trainings
- Completes order entry for diagnostic procedures
- Enters new cases and makes adjustments to the recall list to ensure patients receive follow-up care when appropriate
- Completes program data entry and reporting
- Obtains prior authorizations from insurance for procedures
- Obtains records from the hospital and scans paperwork into eCW
- Completes medication log for stress testing
- Works with Medical, Dental, and Behavioral Health Providers and other staff to ensure patients receive high-quality integrated healthcare
- Acts as a patient advocate for our vulnerable population by ensuring that the patient is fully informed and comfortable with their visits
- Serves as the floor medical assistant to the Site Proceduralist when seeing patients at Salud
- Serves as the secondary assistant to the Site Proceduralist when performing procedures at the hospital
- Completes program data entry, reporting, and invoicing
- Orders all supplies necessary to maintain the program
- Follows procedures for the electronic health records system
- Follows all Standard Operating Procedures that have been defined by Salud as well as any Program associated with Salud
Qualifications
Education:
- High school diploma or equivalent, required
- Medical Assistant, Licensed Practical Nurse, or Registered Nurse credentialed
- Successful completion of a formal MA training program, a minimum of 6 months in length, required
Experience:
- Experience with patient navigation in a cancer screening program preferred
- At least one year of experience working as a medical assistant or nurse in a primary care setting
- Basic Microsoft Office skills in Word and Excel programs, ability to use electronic health record systems
Licensure/Certification:
- Credentialed MA, LPN, or RN
- MA’s not certified/registered upon hire are required to complete the MA certification/registration testing within 3 months of hire. If an employee does not pass, they will be given an additional 3 months to retake the test. All Medical Assistants must be certified within 6 months of hire and certification must be maintained throughout employment.
- Bilingual English/Spanish required
- Excellent verbal and written communication skills in English and Spanish is required
