Position Summary:
Responsible for providing case management services to families of children and youth with Autism and Special Healthcare Needs. Position is hybrid with some travel to client’s home.
Responsibilities:- Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
- Provides case management services to families including interviews to identify and assess their needs and assists them with accessing federal, state and local services and support
- Submits data and reports as necessary to meet reporting requirements of the grant
- Participates in child case finding activities to identify gaps in service delivery
- Integrates new trends and developments in case management
- Adheres to agency infection control and safety policies
- Adheres to the organization’s policy in regards to absenteeism and appearance.
Qualifications:
- BSW or MSW
- Valid NJ SW certification/license (CSW/LSW/LCSW)
- 1+ years’ experience in pediatric case management for children and youth with special healthcare needs
- Strong computer skills
- Excellent verbal and written communication skills
- Strong organizational and interpersonal skills
- Valid NJ driver's license
Working Conditions/Physical Demand: Position is hybrid. Some work will be remote but visiting with clients in their homes or community as well as coming into the office is sometimes required.
