Facility Social Worker

RESPONSIBILITIES:

• Ensures resident adjustment to rehabilitation or long-term care by performing cognitve testing, initial plan of care, care planning and completing all necessary paperwork including MDS.
• Reviews Resident Rights with all new admissions.
• Ensures proper paperwork is completed. 
• Schedules interdisciplinary team meetings with medical staff, residents and family members in order to plan care and resolve concerns. Write reports and ensures implementation.
• Demonstrate understanding and provide guidance and counseling to residents and their families on diverse issues ranging from grief and loss to acceptance and coping with changes in living arrangements.
• Coordinating discharge services - planning and acquisition of appropriate follow up care

QUALIFICATIONS:

• Long Term Experience: 1-2 years minimum
• Bachelor's Degree in Social Work, Psychology or Related Field
