Summary:
Provides assessment, planning, counseling and education to patients.
Responsibilities:
Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives. Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.Supervises and/or trains new staff, students, and interns.Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.

In all respects, verbally and behaviorally, must support Memorial Healthcare System's mission and values.Must demonstrate the ability to provide social work services to patient, family members and significant others in four areas: 1) psychological functioning, 2) adaptation to illness, injury, disability or life event, 3) psychosocial assessment/evaluation and 4) facilitation services such as referral community agencies financial planning, and provision of information and resource knowledge. Required to identify and intervene for suspected abuse including sensitivity to age specific concerns; physical/emotional/sexual abuse of children and adolescents; domestic abuse for adults; and caretaker/institutional abuse for senior adults. Must be able to manage crisis situations, demonstrating ability to quickly appraise the crisis situation and select appropriate intervention. Able to document direct service in patient's medical record after each significant encounter. Must demonstrate proficiency in use of the electronic medical record. Must be able to demonstrate good problem-solving skills, root-cause analysis, and principles of clinical performance improvement. Required Work Experience: One (1) year social work experience in a health care setting.
