P.S. - If residing outside Massachusetts, willingness to obtain a Massachusetts license is required (expenses covered by the client).
Remote Position - Anywhere in the United States
The position of Licensed Social Worker involves working under the supervision of the Clinical Excellence Manager. Responsibilities include delivering evidence-based interventions utilizing the Collaborative Care Model to individuals across pediatric and/or adult populations with mild to moderate symptoms.
Your Tasks:
- Customize treatment plans for patients based on the needs of the assigned Care Pod.
- Coordinate closely with primary care providers and other relevant service providers for comprehensive behavioral health care.
- Conduct screenings and assessments for common behavioral health and substance use disorders using standardized tools.
- Monitor patients for changes in clinical symptoms and treatment side effects, aiming for symptom reduction.
- Support psychotropic medication management prescribed by primary care providers.
- Track patient follow-up and clinical outcomes using a registry system.
- Collaborate with patients to establish self-management goals and interventions.
- Initiate care plan changes for patients not showing improvement, including medication adjustments or referrals.
- Document patient progress and treatment recommendations in designated systems.
- Develop relapse prevention plans for patients in remission.
- Educate patients about behavioral health, substance use disorders, and treatment options.
- Establish care management plans in collaboration with the care team and patients/families.
- Participate in caseload consultations with psychiatric consultants.
- Identify social determinant barriers and assist in providing access to appropriate care.
Requirements:
- Master's degree in social work, counseling, or related field with independent licensure (LCSW/LICSW, LMSW).
- Proficiency in evidence-based treatment approaches such as Problem-Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, and Behavioral Activation.
- Ability to handle higher acuity patients/situations.
- Technical proficiency in various platforms including EHRs with a willingness to learn new technology.
- Familiarity with behavioral health screening tools like PHQ9, GAD7, CSSRS, etc.
- Fluency in both English and Spanish is advantageous.

60,000 - 65,000 /year
