Plans, implements and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients, linkage to resources in the community.
Qualifications:In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:
- MSW (in Social work - Required)
- Licensed Clinical Social Worker (LCSW) preferred. Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) from American Heart Association required for facilities with an Automated External Defibrillator (AED) preferred for facilities without an AED.
- Current Driver’s license
Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10
lbs. constantly
This part time benefit eligible role covers shifts 8:00am - 4:40pm. We do rotate through weekends and holidays with this role. This is a medical Social Worker position working with patients on the medical floors, helping them to access community supports and resources for post DC sustainability. It includes biopsychosocial assessments, resources, placement in SNFs, HHC, long term nursing placement. Also assists with advanced care planning, and care goals discussions. The role coordinates with interdisciplinary teams, RN Case Manager, bedside nurse, MDs/DO’s and community partners.

Pay Range:$30.70 - $49.16 /hour
