The function of the Lanakila Early Childhood Services Program is to provide comprehensive diagnostic evaluation, treatment and follow-up for children from birth to three years old, who are developmentally delayed/disabled, and assists families in developing their knowledge, skills and attitudes to facilitate optimal growth and development of child and family

The purpose of the position is to support the provision of culturally competent, family-centered social work and care coordination, and is assigned a caseload predominant in highly complex and intensive cases

The position is a member of a transdisciplinary team (e.g., speech pathologists, occupational therapists, physical therapists, special education teacher, etc.) and performs work in the childs natural environments (e.g., home, school, community, etc.)

Collaboration is frequent with a variety of multidisciplinary teams, in public and private agencies, and community groups

The position may be assigned to work in an early intervention program within the state to permit optimal flexibility at the State level to respond to emerging and shifting early intervention needs as they arise

MAJOR DUTIES AND RESPONSIBILITIES A

Care Coordination 40% 1

Schedules, facilitates and participates with the transdisciplinary team, in the development of the Individualized Family Support Plan (IFSP) with families and other service providers in compliance with Part C and State guidelines

This will also include review and annual IFSPs and any additional meetings to support the family e.g., meetings to discuss behavioral support services

Serves as a liaison between family and appropriate hospitals, medical providers, public/private agencies, service providers, advocacy programs, health insurance providers, and parent groups to integrate Position Number 26202 Page 2 of 8 and coordinate care with IFSP activities

This involves establishing and maintaining good working relationships and meeting with all pertinent parties as needed

Coordinates and monitors the implementation of the services and activities identified on the IFSP utilizing family-centered care principles through active discussion and information sharing/exchange with families and pertinent service providers

Coordinates and schedules additional evaluations/assessments and periodic evaluation tool updates in collaboration with IFSP team members

Interprets and clarifies any additional evaluation/consultation results as determined by the IFSP team

Discusses the findings and recommendations for services with the family with sensitivity to their emotional impact

Serve as a liaison between the evaluator/consultant and family

Facilitates, participates and monitors the development and implementation of transition plans as indicated on the IFSP for infants and toddlers exiting Early Intervention

Transition planning includes the coordination of transition conferences, sending of the transition notice, collaboration with possible future service providers, e.g., Department of Education (DOE), agencies in the community, and/or out-of-state agencies

Collaboration with DOE may include attending Student Support Team (SST), Eligibility Conferences (EC) and Individualized Education Program (IEP) meetings

With the familys permission, assists them in the identification and linkage to culturally appropriate resources available in the community

Coordinates the arrangement and payment of Early Intervention services through contracted fee for service providers when appropriate

Initiates the timely completion and distribution of the multiple forms, consents and notices required by Part C of the Individuals with Disabilities Education Act (IDEA)

Gathers and compiles statistical data to measure the effectiveness and benefits of Part C services in Hawaii, which include family surveys, Child Goals Summary Form, etc

Intake and Assessment 30% 1

Reviews assigned referrals (from H-KISS, parents, physicians and other community agencies) for cases that manifest characteristics of high intense complexity; participates in the evaluation of infants and toddlers with special needs to determine eligibility for early intervention services

Initiates contact with the families through telephone and/or home visit to orient family of early intervention philosophy, services and program options

Completes comprehensive intakes, which includes verifying referral information and gathering relevant medical (birth history, diagnosis, vision, hearing, physical and mental health), social (relationships/supports), environmental (living/housing, economic/financial, education), and developmental (including functional domains) information about the children and families

Identifies presenting concerns and priorities and informs families of potential services and/or resources

Begins building collaborative relationship with families and answers any questions to clarify procedures, including introduction to the evaluation tools for eligibility

Attends to immediate needs for services/resources (e.g., housing, medical insurance, transportation, and crisis intervention)

Schedules multidisciplinary developmental evaluations (MDE) and/or gathers the necessary information for a multidisciplinary evaluation

Schedules any other relevant evaluation(s) needed to determine eligibility

Participates in the MDE as a second evaluator as needed and assists in interpreting the findings

Facilitates determination of eligibility, including results of the MDE and pre-existing concerns, such as biological risk factors

Completes the Family Directed Assessment (FDA) with families

This includes a social summary describing familys strengths, concerns and priorities

Family Support Services 20% 1

As appropriate, provides family support services for selected families

Provides support to families through individual, family, and group counseling

Independently provides intensive casework or clinical social work services to families to address difficult and complex (social, physical, and emotional) issues that may arise as a result of extraordinary caregiving demands placed upon family members who care for infants and toddlers

Provides crisis intervention to family members as needed

Provides outreach to family members to encourage parent to parent networking and participation in parent support groups

Provides parenting support and education individually or in groups

Provides on-going support to families to promote participation in services
