We are seeking a Full-Time Home Health Social Worker to join our team. This role involves providing a variety of social services to patients and their families, addressing social, emotional, and economic challenges related to illness or injury. Key responsibilities include crisis intervention, discharge planning, continuity of care, financial assistance, and referrals to community resources, while collaborating with an interdisciplinary team.

Education: Master’s Degree in Social Work required; other healthcare-related fields may be considered.

Experience: Minimum of one year of social work experience, preferably in healthcare.
