Dignity Health Sierra Nevada Memorial Hospital is a 104-bed not-for-profit hospital located in Grass Valley California. The hospital has been providing compassionate quality health care to residents and visitors of western Nevada County since 1958. As an affiliate of the nationally recognized Dignity Health system we ensure the highest standard of health care to our patients and have access to important regional resources throughout the system including the Dignity Health Heart & Vascular Institute the Dignity Health Neurological Institute of Northern California and the Dignity Health Cancer Institute of Greater Sacramento. Responsibilities
The incumbent, as a member of the multi-disciplinary team, provides the following services to patientsand/or families: psychosocial assessment,crisis intervention, counseling, exploration of effective coping skills, crisis intervention, education, and coordination of the patients progress through the continuum of care. In addition, the incumbent provides education, consultation and support to hospital staff.
Qualifications
Required:
+ At least one year of clinical and/or medical social work experience required
+ Master's Degree from a School of Social work accredited by the Council of Social Work Education
Special Skills:
Knowledge of contemporary behavioral and systems theories relevant to health care; end-of-life dynamics and interventions; grief and bereavement counseling; substance abuse identification and interventions; support for victims of abuse, neglect or violence; community or financial resources for underserved or vulnerable populations. SSDI, SDI, and other governmental/county requirements/programs. Thorough comprehension of OBRA and Title 22 is required in California. The incumbent must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable people with disabilities to perform essential functions. Essential functions include but are not limited to those functions that are performed frequently, require a significant amount of time or if not performed, result in an adverse impact.
Pay Range
$40.66 - $58.96 /hour
