Social Worker - Skilled Nursing Center of Allegheny County

We are a skilled nursing facility committed to providing exceptional care to our residents and creating a positive environment for our staff. We are currently seeking a compassionate, dedicated Social Worker to join our team and make a meaningful impact in the lives of our residents.

Job Description:
As a Social Worker, you will work directly with residents and their families to provide social services and emotional support, ensuring a high level of care and well-being for each resident. You will collaborate with the interdisciplinary team to create personalized care plans and assist with discharge planning, advocacy, and crisis intervention.

Social Worker Responsibilities:

• Assess residents' psychosocial needs and work with families to address concerns.
• Develop, implement, and update individualized care plans in collaboration with the care team.
• Provide counseling and support services to residents and families.
• Assist with discharge planning, ensuring smooth transitions and appropriate community referrals.
• Advocate for residents' rights and ensure compliance with facility policies and regulatory standards.
• Participate in care plan meetings and other interdisciplinary team meetings.
• Maintain accurate and timely documentation in compliance with facility and regulatory standards.

Social Worker Qualifications:

• Current state licensure as a Social Worker. 
• Previous experience in a skilled nursing or long-term care setting.  
• Strong communication, organizational, and problem-solving skills.
• Ability to work independently and as part of a team.
• Empathetic, compassionate, and committed to enhancing the quality of life for residents.
