Social Worker

Cortlandt Healthcare is looking for talented social worker to join our growing team.

Social Worker Duties Include:

The Social Worker is responsible to facilitate discharge planning and also provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental, and psychosocial well-being.

·      Provides support to the residents and families in coordinating residents needs to ensure residents needs and expectations are met during their stay at (Facility) and their expectation in the future.

·      Work closely with the interdisciplinary team in coordinating the care plan and discharge plan.

·      Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay.

·      Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community.

Social Worker Requirements/Qualifications:

·     Bachelor’s degree in Social Work or similar professional qualifications with 5+ years experience; working directly with geriatric clients in a health care setting. Experience with MDS, CCP, and Discharge planning

·     Master’s degree in Social Work and/or Social Work license as regulated by NY state law with 1 year experience working directly with geriatric clients in a health care setting. Experience with MDS, CCP, and Discharge planning.

·     60 bed case load

·     1-2 years experience in Nursing Home 

·     LMSW, but MSW with experience is ok

·     Ability to wrok flexible schedule.

·     Experience with PCC is a plus

Salary: Will be offering UP TO $30.00 an hour.
