Social Worker - Case Manager

MUST:
Experienced Case Manager
2+ years of experience as a Case Manger
2+ years of past experience working with persons living with HIV or at high risk of HIV acquisition preferred
Outreach experience and knowledge of community resources a plus
Must be a skillful interviewer, able to ask questions and effectively use interview skills to elicit needed information from clients.
Must be a skillful listener, able to understand the meaning of client's statements.
Must be a skillful observer, able to record the client's behavior and needs.
Must be a skillful negotiator, able to reach definitive agreements with clients and service providers.
Must be able to work well with interdisciplinary teams.
Proficient in most products of Office 365 (Word, Excel, Power Point and Teams)
Ability to prioritize and problem solve to maintain a case load.
Strong oral and written communication skills required.
Good interpersonal and organizational skills required.
High School diploma or GED required.
Associate s/Bachelor s degree in health or human services related field preferred.
Fluency in Spanish highly preferred
DUTIES:
Increase access to support services to promote and support self-reliance and improve individual navigation of health and human service support systems.
Provide guidance and assistance in accessing medical, social, community, legal, financial, and other needed services.
Benefits counseling that assists eligible clients in obtaining access to other public and private programs for which they may be eligible, such as Medicaid, Medicare Part D, State Pharmacy Assistance Programs, Pharmaceutical Manufacturer s Patient Assistance Programs, other state or local health care and supportive services, and health insurance plans through health insurance
Initial assessment of emergent service needs, and appropriate referrals
Development of a comprehensive, individualized care plan
Continuous client monitoring to assess the efficacy of the care plan
Re-evaluation of the care plan at least every 6 months with adaptations as necessary
Ongoing assessment of the client s needs and personal support systems
Update progress notes documentation in the client paper chart/EHR, on the required forms and in the required CARE Ware Ryan White database.
Participate in or lead Quality Management and Quality Improvement projects
Participate in monthly Case Management meetings conducted via Zoom.
Performs other duties as assigned.
