Social Services Manager - Courtland - CaroMont Health

Job Summary:  Provides psychosocial assessments, plans and implements various counseling, conducts and coordinates discharge planning for complex cases and participates in multidisciplinary team process. Victims of domestic violence, adult neglect, and abuse, financial fraud and other DSS issues shall be the Social Workers responsibility. The discharge planning service integrates the clinical with the financial, Psychosocial and legal components, and identifies internal and external barriers that prevent desirable resident outcomes. The Social worker shall provide counseling, education and resource identification while providing quality service. Must be knowledgeable with State and Federal laws as they pertain to adults as well as the elderly. The Social worker will act as a resource consultant for the Discharge Planning with the physicians in their discharge planning role. 

Qualifications:   Bachelor degree in social work or human service field required (Master’s preferred) with 5 years experience in health care or long term care field.   Extensive experience preferably in the community and long term care setting. Thorough knowledge in principal and methods of social work counseling, including guardianship issues, neglect and abuse issues, and financial concerns for the elderly. Knowledgeable of state and federal laws as they relate to social work reporting practices. Exemplary verbal and written communications, creative thinking and cost saving practices. Proficient with computers and MS office software.
