Responsibilities and Purpose
- Identify each resident’s social, emotional and psychological needs.
- Develop and carry out a plan to develop the residents’ full potential during their stay.
- Planning for the resident’s discharge or transition to another care environment.
Desired Experience and Education
- Bachelor’s degree in social work OR Bachelor’s degree in human services field including, but not limited to, sociology, special education, rehabilitation counseling, gerontology and psychology.
- Completion Certificate for Oklahoma-approved Social Services training program.
- Knowledge of federal and state guidelines.
- One (1) year of supervised social work experience in a healthcare setting working directly with individuals.
- One (1) or more years or work in a skilled nursing facility, hospice or long term acute care.
