FUNCTION: Responsible for planning, developing, organizing, implementing, and directing the SNF Health & Wellness in accordance with current existing federal, state, and local regulations, as well as Stellar Senior Living established policies and procedures, to ensure medically related social services are provided to all residents in the facility.
POSITION SPECIFICATIONS: The following specifications may differ depending upon the number of available employees among whom the performance of a particular job function can be distributed and the size and type of community.

QUALIFICATIONS:
• Persons who have been found guilty by a court of law of abusing, neglecting or mistreating individuals in a health care setting are ineligible for employment in this position.
• At least one year supervised experience in a health care setting working directly with clients/residents.
High School Diploma or equivalent, Associates Degree in Human Services Preferred. Facilities with 120 beds or more must have a bachelor’s degree in social work or human services.
• Working knowledge and basic competency with a personal computer and basic software applications used in job functions such as Microsoft Word.
• Demonstrated leadership skills applicable to a role in an organization such as a long-term care facility.
• Strong organizational and analytical skills; oral and written communication skills.
• Working knowledge of and ability to apply professional standards of social work practice in job situations.
• Current knowledge of state and federal laws and regulations that apply to the practice of social work in long-term care.
• Current Social Work license or certification as required by state regulation.
• Must be able to express self adequately in written and oral English communication and to communicate effectively in an interdisciplinary setting with residents, families, staff members, representatives of the community and visitors.
• Ability to add, subtract, multiply, and divide in all units of measure of weights, using whole numbers, common fractions and decimals.
• Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
• Ability to analyze data, reports, business documentation and problems and develop workable solutions and goals.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
• Plan, develop, organize, implement, evaluate, and direct the social service programs of the facility.
• Assume the authority, responsibility, and accountability of directing the Health & Wellness Department.
• As part of the admission process, meet with the resident and/or family to review policies on advanced directives, resident rights, and discrimination.
• Conduct and document a psycho social history and assessment, including identification of resident problems/needs.
• Provide for and/or arrange for social work services as indicated by resident needs.
• Communicate with resident’s family about resident needs, transfers, discharge and changes in level of care, and financial responsibility.
• Maintains a current list of community resources to facilitate referrals, for residents in the facility, and at time of discharge.
• Able to establish criteria to assess those services provided meet established standards of care.
• Conducts social service groups. • Provides planning and implementation of one-to-one interventions.
• Provides advocacy services to residents.
• As part of the interdisciplinary team, develops behavior interventions and oversees facility behavior committee.
• Prepares measurable care plan goals for the residents.
Job Description
• Demonstrates satisfactory level of interpersonal skills to interact with residents, families, interdisciplinary staff, regulators, and the community.
• Demonstrates assessment skills sufficient to evaluate residents’ behavior, psycho social needs, and risk factors for psycho social deterioration and residents’ response to interventions.
• Effectively implements social service interventions that achieve goals, address resident needs, and link social supports, physical care, and the physical environment to enhance quality of life.
• Interprets and explains resident behavior to staff to foster understanding, facilitate treatment and foster respect for resident rights.
• Coordinates facilities efforts to reduce chemical and physical restraints. • Demonstrates knowledge of and respect for the rights, dignity, and individuality of each resident in all interactions.
• Reviews and develops a plan of correction for social service deficiencies noted during survey and provide a written copy of such to the administrator.
• Reviews departmental complaints and grievances from residents/families etc. and provide reports to the administrator indicating what action(s) were taken to resolve the complaint per Stellar Senior Living policies and procedures.
• Carries out all duties in accordance with the facility mission and philosophy.
• Complies with facility policies and procedures for workplace safety including infection control procedures, application of universal precautions for blood borne pathogens, use of personal protective equipment and handling of hazardous materials.
• Prioritizes tasks/responsibilities and complete duties/projects within the allotted time.
• Ensures that the quality and appropriateness of Resident care meets or exceeds company, industry and regulatory standards and ensure services are in compliance with state and federal legal, regulatory, accreditation, and reimbursement guidelines.
• Maintains effective relationships and open communication with Residents, families, and staff.
• Participates in quality improvement activities.
• Must be able to carry out duties in a setting that may provide multiple interruptions.
• All other related duties as assigned.
