The Social Services Director/Admissions Coordinator is a key member of the Lakeside Health and Rehab Leadership Team who plans, organizes, develops, and directs the overall operation of the Social Services department to ensure all medically related emotional and social needs of patients are met in accordance with all applicable laws, regulations, and Lakeside standards. The Admissions Coordinator facilitates the conversion of referrals to admissions through the completion of the intake process and the use of effective marketing and customer service skills.
Your Job:
- Manage and direct the Social Services Department, and identify and provide for each residents social, emotional, and psychological needs.
- Understand the MDS process with ability to interview, input, and complete social service assessments.
- Accurately assess, document and provide interventions to meet psychosocial needs of all residents.
- Develop care plans based on assessments
- Maintain a quality working relationship with the medical profession and other health related facilities and organizations.
- Manage department to assure all psychological and psychosocial needs of residents are met.
- Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Administrator as required.
- Provide effective facilitation of resident care conferences: schedule, send letters to families/ responsible parties, and participate in care conference.
- Facilitate all resident discharges (arrange for home health, DME, assist with transfer to other facilities)
- Answer and respond immediately to inquiries concerning potential admissions.
- Provides support and assistance to the resident and family during the admission process.
- Assist with facility marketing initiatives.
- Create relationships with referral sources.
Who You Are:
- Two years of job-related experience, preferably in long-term care (Have other qualifying experience? We want to hear from you!)
- Experience with social work, preferably working directly with patients and their family members at a skilled nursing facility
- Detail oriented, computer literate and have excellent communication skills
- A professional team player with a commitment of maintaining a reliable shift history
- Passionate about contributing to an organization focused on providing genuine hometown care
