Social Services Assistant
Location: San Jose, CA
Pay: $23-$29/hour (depending on experience)
Schedule: Full-time, M-F 8:30am - 5:00pm

In this pivotal role, you'll be instrumental in shaping and executing our social service programs. From planning to evaluation, you'll work tirelessly to ensure that our residents' emotional and social needs are not just met, but nurtured to the highest standards. Operating in alignment with federal, state, and local regulations, you'll be at the heart of our mission to provide personalized care on every level.
Duties and Responsibilities:
- Assist in planning, implementing, and evaluating social service programs.
- Collaborate with staff and departments to address residents' social and emotional needs.
- Develop and implement policies for identifying residents' social needs.
- Aid in the admission process and discharge planning.
- Obtain and assess social and family information for treatment planning.
- Maintain detailed progress notes and perform administrative tasks.
- Arrange appointments and transportation for residents and families.
- Coordinate with community agencies to meet resident needs.
- Provide information on financial assistance programs to residents and families.
- Resolve grievances and address emotional concerns of residents and families.
- Assist in finding solutions to social and practical issues, including discharge planning.
- Interpret residents' social and emotional needs to the care team.
- Participate in Resident and Family Group meetings as needed.
Preferred Qualifications:
- Bachelor's degree in Social Work, Psychology, Sociology, or related field.
- Previous experience in a social services role within a healthcare or long-term care setting.
- Familiarity with community resources and agencies for referral purposes.
- Certification or licensure in social work or related field.
Required Qualifications:
- High school diploma or equivalent.
- Minimum of one year of experience in a social services role.
- Knowledge of federal, state, and local regulations governing social services in healthcare facilities.
- Willingness to undergo background checks and comply with regulatory requirements.
