Social Services Assistant - Long Term Healthcare Facility

University Nursing and Rehab is seeking a caring and passionate full-time Social Services Assistant to join our social services team.

DUTIES:

• Identifying individual social and emotional needs
• Developing and Maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

REQUIREMENT:

• Previous experience working in long-term care is preferred
• Bachelors degree in Sociology or Physcology is preferred
