Lakewood Rehabilitation and Healthcare Center is now hiring a full-time Social Service Director. Lakewood Rehabilitation and Healthcare Center is an 110-bed skilled nursing facility with private and semi-private rooms located in Nanticoke, PA . Knowledge, Skills and Abilities
Requires demonstrated professional knowledge and skills necessary to plan, organize, and develop support services for our current residents and their families; the ability to establish and maintain effective professional working relationships with residents, families, and staff; knowledge of the inquiry and admissions process; fundamental knowledge and skill in marketing; effective verbal and written English communication; lift and carry up to 30 pounds.
Education
Graduate of an accredited institution with a bachelor’s degree in social work or extensive equivalent experience in the field.
Experience
Prior social work experience in the geriatric field preferred.
Certificate/Licenses
Bachelor’s Degree in Social Work field under the regulation with proper experience, valid Pennsylvania driver’s license.

Job Decription
1. Conduct continuous family and resident counseling including crisis intervention, facility adjustment, resident status changes, etc.
2. Assess each resident within seven (7) days of admission and on a quarterly basis.
3. Attend monthly Resident Council meetings (when invited by Council) and address resident concerns.
4. Coordinate room changes; notify residents & families.
5. Coordinate changes such as room transfers, cable and telephone, pharmacy, laundry, etc.
6. Assist discharged residents and families with placement options.
7. Coordinate Discharge Planning Service (i.e., discharge time, special needs) and initiate a discharge follow-up call within forty-eight (48) hours after discharge.
8. Check PASSAR on admission & complete 408’s on targeted residents.
9. Direct and assist Admissions Coordinator with inquiries and facility tours, marketing, and networking.
10. Assist with admission sign-in and discharge process as needed.
11. Establish and implement a follow-up visit system for all new residents per individual care plans.
12. Visit all new residents and families within 24 hours of admission to facility.
13. Participate in quarterly Care Plan Conferences, including invitations to families and follow-up review with residents. Assure good representation at care conferences.
14. Complete MA 51s for residents being transferred to an ALF.
15. Participate in weekly Care Navigation Meetings.
16. Participate in monthly Behavior Management meetings and follow up with any concerns.
17. Practice safe working habits.
18. Update Department’s Policies and Procedures manual at least annually.
19. Participate in Marketing activities
20. Identify Target residents/408; complete reporting forms within 48 hours of admission.
21. Write necessary plans of care.
22. Address resident and family concerns and/or complaints.
23. Complete assigned MDS sections.
24. Complete PA-PASRR-ID forms within 48 hours of admission.
25. Complete Notice of Non-Coverage letters and issue to residents/families.
26. In facilities with one or more social services, the Director will ensure that the above is completed and lead the department. All the above are to be completed by social services.
27. Other duties assigned at the site level
