Social Service Director - Blue Palms Health and Rehabilitation Center of Deland

Blue  Palms Health and Rehabilitation Center of DeLand is seeking a dedicated and compassionate Social Service Director to join our team. As a Social Service Director, you will be responsible for overseeing and managing the social service programs and initiatives within our organization. You will play a crucial role in assessing the needs of individuals and implementing strategies to improve their overall well-being. Responsibilities

• Develop and implement social service programs and policies that meet the unique needs of our clients
• Coordinate and monitor the delivery of social services provided by our team of dedicated professionals
• Conduct assessments to identify the needs and goals of individuals and develop personalized care plans
• Collaborate with other departments and community agencies to ensure a holistic and comprehensive approach to service delivery
• Provide guidance and support to social service staff, including training, supervision, and performance evaluations
• Stay up-to-date on the latest research, best practices, and regulations related to social work and implement necessary changes and improvements to our programs
• Maintain accurate and up-to-date records and documentation of all social service activities and interventions

• Bachelor's degree in social work or a related field (Master's degree preferred)
• Proven experience working in a social service role in a Skilled Nursing Facility( SNF)
• Strong knowledge of social work principles, practices, and ethics
• Excellent interpersonal and communication skills
• Ability to work collaboratively with a diverse team
• Strong organizational and time management skills
• Proficiency in computer applications and case management software
