Key Responsibilities:
As a Social Services Assistant, you’ll have the unique opportunity to make a real difference in the lives of our residents by:
- Assessing Needs: Complete assessments for each resident, identifying their social, emotional, and psychological needs.
- Collaborating on Care Plans: Actively participate in creating and updating the interdisciplinary care plan to ensure a comprehensive approach to each resident's well-being.
- Providing Support: help residents cope with the transition and adjustment to a long-term care environment, ensuring they feel supported throughout their stay.
- Family Support: Provide education to residents and their families, helping them navigate challenges and maintain strong connections.
- Discharge Planning: Coordinate the discharge process, ensuring appropriate home care services are in place for residents’ safe and successful transitions back into the community.
What We’re Looking For:
- Educational Requirements: Associate’s or Bachelor’s degree in Social Work, Psychology, Sociology, or a related field preferred.
- Experience: Previous experience in social services or healthcare, especially working with elderly or disabled populations, is highly desirable.
- Skills & Qualities:
- Strong interpersonal and communication skills
- Empathy and compassion for working with the elderly or individuals with disabilities
- Knowledge of federal and state regulations regarding long-term care
