Social Determinants of Health (SDOH) Manager - Full Time

As our SDOH Manager, you will lead organizational efforts to identify and track the most frequently experienced social needs of the people being served at SouthLight. The manager will develop collaborative relationships with community organizations so as to create pathways and links for clients trying to access services addressing their social needs. In addition, this position will work with staff to support program participants gaining access to the services they need in order to meet their goals and thrive. This may include educating existing staff on the resources available as well as assisting with navigation of services for the most acute patients.
The goal of this person's work will be to show how SDOH interventions can support recovery and improve health. Successful candidates will demonstrate knowledge of community resources and the ability to secure such resources to benefit SouthLight's participants. They will work alongside the clinical and medical team and be a support to them and patients. They will assist SouthLight's efforts to develop a process for managing its patient population, collecting data and connecting our work to improved participant outcomes.
Schedule
Full-time days, 40hrs/wk.
Pay
$55,000 + per year.

You will be eligible for medical benefits the first of the month following your first 30 days. We have options for a standard plan, as well as an HSA with a $300 initial contribution and up to a $25 per paycheck match. You would receive 10 paid holidays as well as PTO hours which would accrue at a rate of 10 hours per month. After 6 months you would be eligible to enroll in our 401(k) plan which has a 4% match. We also provide company paid life insurance, short- and long-term disability, in house and paid training opportunities, and paid credentialing. SouthLight is also an approved employer for the Public Service Loan Forgiveness Program (PSLF).
Job Responsibilities of a SDOH Manager
- Be a resource to SouthLight staff and help train staff on resources available to participants; be a coach on referral materials and processes and help provide ways to reduce barriers to linking clients with needed services
- Support the establishment and administration of screening tools that will identify the social needs of SouthLight participants
- Produce reports and review data to monitor and identify the most pressing social needs of SouthLight participants and use the data to inform SouthLight's work to address social needs
- Be familiar with key points of contact and referral processes at community organizations so that SouthLight can link participants to programs that will meet their needs and be an advocate for SouthLight participants to receive services
- Coordinate with community organizations on behalf of SouthLight for letters of support in order to access grants, contracts and other resources related to SDOH
- Be a collaborative partner on events and volunteer projects related to meeting the social needs of program participants; i.e. assisting with Recovery Month and/or volunteer food/kit drives
- Determine relevant data and collect/report data related to the types of services needed by participants and the referrals and connections that are made by SouthLight with community organization's offering those services
- Work with the Organizational Excellence team on initiatives to identify and track the health benefits to SouthLight's participants who are connected to social programs and help tell a story about how interventions lead to better health outcome
Qualifications of a SDOH Manager
Education and Experience
- Qualified Professional - Bachelor's degree in Human Services
- Experience working with the mental health and/or substance use population or in related field.
Knowledge, Skills and Abilities
- Strong written and verbal communication skills
- People skills including influencing, delegation, teamwork, supervision and team management
- Ability to work effectively with a diverse staff, clients, and community members in a culturally competent manner
- Flexibility and adaptability - managing multiple priorities in a dynamic and fast paced work environment.
- Effective representative to external partners, ability to prioritize organizational goals while initiating and influencing new relationships
- Collaborative approach to leadership and coalition building, works well in a team and across various areas of an organization
- Experience with community-based organizations; knowledge of resources available in the community related to housing, transportation, food insecurity, inter-personal violence and more.
- Familiarity with needs of people in recovery and receiving substance use and mental health services
- Organizing SDOH data, e.g., plan, administer, report and analyze SDOH data
