About the Role
We are seeking dynamic and dedicated Social Care Specialists at both Associate and Senior levels to join our team. Ideal candidates will bring experience in care planning or care management, or demonstrate a strong willingness to develop these skills. In this role, you will play a key part in supporting the daily operations of our programs, working closely with our Social Care Manager. This hands-on position requires in-depth knowledge - or a commitment to learning - case management, Medicaid, and other government benefits.
Qualifications:
- Bilingual: Spanish is required, with fluency or advanced-level language skills.
- Bachelor’s degree in Human Services or a related field, or equivalent work experience.
- Minimum of 2 years of experience in care planning, care coordination, or care management.
- Demonstrated experience in housing support, government benefit applications, and managing complex cases.
- Strong judgment and decision-making skills to advocate effectively for clients’ best interests.
- Proven ability to work collaboratively with individuals and groups from diverse backgrounds.
- Comfortable conducting outreach and cold calls in medical or community settings.
- Exceptional interpersonal and communication skills, with the ability to build rapport with individuals from varied professional and personal backgrounds.
- Must maintain current licenses and certifications required for the position, including (but not limited to) First Aid and CPR.
- Ability to pass pre-employment and random drug screenings while maintaining professionalism, emotional control, and composure.
- Reliable access to a vehicle, proof of insurance, and a valid driver’s license for essential job-related travel.
- Proficient in computer applications, including electronic health record (EHR) systems.
- Excellent oral and written communication skills.
Responsibilities:
- Manage a caseload of complex cases, ensuring clear communication with patients and caregivers, accurate documentation in EHR systems, and timely data entry, maintenance, and reconciliation.
- Provide oversight, guidance, and support to the Social Care team to ensure effective operations.
- Connect clients to appropriate community services by offering warm handoffs and providing comprehensive referrals.
- Respond promptly to voicemails and emails from participants, community partners, and other stakeholders.
- Facilitate seamless navigation across sectors and providers to ensure coordinated service access and completion for all cases.
- Conduct pre-screening for client eligibility, assist with applications, disseminate essential information to clients and partner organization staff, and track data for follow-up.
- Collaborate with the Social Care Manager to expand outreach efforts and enhance residents’ awareness of available resources and services.
- Utilize and maintain up-to-date knowledge of government benefits, policy updates, and program procedures to serve as a trusted resource for clients and colleagues.
- Participate actively in internal meetings to stay informed of program updates and operational developments.
- Maintain meticulous notes on client interactions and ensure thorough follow-up with referral sources.
- Engage in regular meetings with team leads to align on goals, updates, and best practices.
- Prepare and submit statistical data and required paperwork in an accurate and timely manner.
- Execute additional tasks and responsibilities as assigned by leadership to support the organization’s mission and goals.
