Southwest Health System, Inc. has an opening for a full-time Sleep Health Advanced Practice Provider (APP) to join our close-knit team in Cortez, CO. The ideal candidate for this Sleep Health APP position is a Nurse Practitioner or Physician Assistant that is looking to provide direct clinical patient care full time and have the responsibility for referrals to other health care providers. This position will be the point for case coordination among sleep health centers, acute and chronic care facilities, medical equipment providers, community resources, primary care providers, and other health care specialists.

HOW YOU MAKE A DIFFERENCE
The full-time Sleep Health APP position offers you a flexible work schedule. This position will further offer the flexibility to provide patient care within the health system. This is a salary position with many leadership opportunities.
As a Sleep Health APP, you will be a member of the Southwest Medical Group and, as such, will be involved in all the aspects of a thriving primary care and specialty care practice serving the Four Corners region. The Sleep Health APP directs Southwest Health System sleep health patient care under the administrative direction of the Chief Medical Officer (CMO). This position will work closely with health system staff, department directors, and providers to ensure that the highest standards of quality and service are maintained. The Sleep Health APP may also be a practicing provider in their practicing specialty at SHS.
A Sleep Health APP primary roles and responsibilities will include:
- Initial sleep evaluations and follow-up visits.
- Histories and physical examinations.
- Ordering, reviewing, and explaining test results.
- Diagnostic sleep disorders.
- Choosing therapies and developing plans of care, i.e.,
- Positive airway pressure therapy or other devices.
- Medication Management.
- Modifying sleep diagnoses, therapies, and/or plans of care.
OUR IDEAL SLEEP HEALTH ADVANCED PRACTICE PROVIDER
- Physician Assistant Requirements:
- Completion of a certified Physician Assistant or Nurse Practitioner program with a Colorado state and national certification as evidenced by a certificate of Physician's Assistant status by The National Commission on Certification and testing by the State Board of Medical Examiners.
- Current and unrestricted Physician Assistant license issued by the Colorado Board of Medicine.
- Nurse Practitioner Requirements:
- Current unrestricted Colorado registered nursing licenses and Advanced Practice license with a prescriptive endorsement.
- Completion of masters, post-masters, or doctorate degree in an Nurse Practitioner program accredited by the Commission on Collegiate of Nursing Education (CCNE) or the Accreditation Commission for Education in Nursing (ACEN).
- Two or more years' experience in a clinical capacity required.
- Current and unrestricted DEA
- Able to work in a team environment and with your assigned physician supervisor(s).
- Knowledge of electronic medical record systems and applications to clinical settings.
Knowledge of additional languages besides English would be preferred.
