Waypoint is looking for an experienced Shared Living Coordinator in our southern Maine region. If you have supported Shared Living Providers and members using this service please reach out for an interview. We have a dynamic team with fantastic support from management and the community. This position is based out of our Springvale office and covers primarily York County and some Cumberland County areas.
The Shared Living Coordinator is responsible for on-going support and monitoring of the Shared Living Providers (SLPs) and the services provided. The Coordinator is the providers’ primary point of contact for Waypoint, offering them guidance and information to successfully support the individual(s) for whom they are providing services. This position works closely with the Shared Living Manager to ensure the services are compliant with Waypoint standards and the Department of Health and Human Services rules. The Shared Living Coordinator works closely with the Shared Living Providers to manage their training, complete their daily documentation, and submit Invoice to ensure timely payment for services, develop the plans for the member(s), access respite if needed, and best meet the needs of the member(s) within their care.
Responsibilities:
- Support the Provider in the member’s PCSP meetings, including developing the Service Implementation Plan (SIP) with the member and SLP, submitting the plan I the Evergreen System.
- Ensure implementation of a member’s PCSP, monitoring progress toward goals and overall quality of life experienced by the member.
- Communicate and interact collaboratively and professionally with members, providers, family members, guardians, case workers, and other stakeholders.
- Understand and comply with the Home and Community Based MaineCare Rules and ensure best practice and quality standards in service delivery.
- Ensure the provision of coordination and advocacy for access to services, supports, and resources.
- Provide consultation, support, and mediation to Shared Living Providers and Members as needed.
- Complete all necessary documentation and reports to ensure timely submission of all documents to stakeholders according to the standards of the agency.
- Adhere to a regular schedule of bimonthly home visits with phone contact during the month that the home is not visited.
- Maintain accurate documentation of home visits, home inspections and contact calls with the Shared Living providers.
- Compare SLP Invoices and daily documentation for billing and payment accuracy before submitting to manager for approval.
- Maintain all SLP and Member records in accordance with the agency standards and the Office of Aging and Disability
- Ensure all OADS and MaineCare requirements are adhered to as related to Shared Living.
- Ensure SLP follow mandated reporter requirements; notifying and documenting events in Therap GER section. SLC will review an ensure complete and accurate information for submission to OADS.
- Complete Quality Assurance responsibilities including Monthly Documentation Audits, Provider and Member file reviews and collect outstanding items.
- Ensure compliance with all Waypoint policies and procedures.
- Fully understand and abide by the agency’s code of ethics.
- Supports Shared Living Providers in all areas related to their responsibilities.
- Ensures that the SLP completes and provides documentation of ongoing professional training requirements according to rule.
- Adheres to all Waypoint safety protocols.
- Ensures the Provider(s) home reflects Waypoint and DHHS standards for a safe living environment. Shares any concerns with the living space with the Shared Living Manager in a timely manner.
- Maintains the agreed upon work schedule, notifying their supervisor of any necessary changes.
- Completes all required training for the position in a timely manner and maintains credentials.
- Attend and participate in Monthly Team/Staff meetings.
Requirements
- Experience and knowledge of the Shared Living Option preferred.
- Experience working with individuals with a diagnosis of Autism, Intellectual Disabilities, and mental health challenges.
- Ability to remain supportive and empathetic in all situations.
- Ability to remain flexible and quickly adapt to changing circumstances.
- Must be able to problem solve and offer solutions to challenging situations.
- Ability to speak, read and write in English.
- B.A. Degree preferred. Degree in Social Services or a closely related field preferred.
- Valid driver’s license required.
- Must have a reliable vehicle with proper insurance, current registration, and a valid driver’s license from state of residence.
- Some early evening hours or weekends may be required to meet with Providers and members.
- Excellent interpersonal and time management skills. Must have professional level writing skills to maintain documentation standards, reports, and any other related responsibilities.
- Effective computer skills including navigating multiple systems and keyboarding.
- While performing this job the employee is regularly required to sit for extended periods of time.
- Ability to travel.
