Service Coordinator Supervisor and/or Director of Service Coordination/Case management
JOB SUMMARY:
The purpose of this role is to provide Service Coordination to approved HCBS TBI/NHTD participants who choose ABBA as their provider agency. Service Coordination is an individually designed service to provide primary assistance to the waiver applicant/individual in gaining access to needed Waiver Services, Medicaid State Plan Services and any other services deemed necessary to assist the individual with maintaining their independence in the Community. The Service Coordinator is responsible for timely and effective implementation of the approved service plan.
RESPONSIBILITIES:
- Complete and submit to the RRDC, the person-centered, individualized Initial Service Plan (ISP) insuring that is maintains the health and welfare of the participant.
- Maintain the waiver program eligibility of the participant.
- Secure initial and annual level of care assessments.
- Assure Team Meetings are scheduled and held as designated in the approved service plan.
- Facilitate the acquisition, oversight, and delivery of service(s) to enhance participant's quality of life, maintain their independence and placement in the community.
- Complete annual services plans (Revised Service Plans/RSPs) and ensure they are submitted to the RRDC a timely manner.
- Complete and update as necessary the participant's Plan of Protective Oversight (PPO) to support the service plan and maintain the welfare and health of the participant in the community.
- Conduct a minimum of one face to face visit with the participant assuring that at least one visit per quarter is held in the participant's home.
- Maintain records for at least six (6) years after termination of waiver services.
- Respond to participant crises and emergencies.
- Address problems in service provision.
Education:
Persons employed as Service Coordinators must be a/an:
- Master's Degree in Social Work, Psychology, or related field.
AND
Must have, at a minimum, one (1) year of experience providing case management/service coordination, information and referral services and service brokering of community based services for individuals with disabilities and/or seniors.
OR
Be an individual with a Bachelor's degree and three (3) years of experience providing case management/service coordination, information and referral services and service brokering of community based services for individuals with disabilities and/or seniors.
Additional Requirements:
- Possess the necessary documentation to work in the United States.
- Demonstrate ethical business practices, in conformance with all state and federal laws and regulations.
- Possess excellent verbal and written communication skills.
- Be able to multi-task and prioritize duties.
- Be able to travel in all weather conditions throughout the counties served.
- Be willing to foster company, department and program wide cooperation and team work through use of positive/constructive communication techniques.
- Be able to maintain absolute confidentiality in regards to all records reviewed including consumer records, employee records and billing records.
- Have proficiency and experience with PC's, Microsoft and Web-based software applications.
- Be able to analyze and interpret data and facilitate issue/problem resolution.
- Be in possession of a valid NYS Driver's license and have a driving record considered acceptable to company and its insurance carrier.
- Have continuous access to a reliable, registered and insured vehicle.
- Demonstrate full adherence to the ABBA Human Services Foundation Code of Conduct and all policies/procedures related to compliance.

- Be able to meet the physical requirements of the position with or without reasonable accommodation:
- Be able to read write and interpret written documents;
- Have an excellent working knowledge of HCBS waiver and community based service programs and working with individuals with disabilities, seniors and/or Traumatic Brain Injury.
- Previous experience working with diverse populations, including low to moderate-income families and people with disabilities a plus.
- Bilingual (Spanish-English; Creole-English) a plus.
