One of our clients is seeking an Senior Social Worker
Type: Contract to Perm
Schedule: Tues - Sat schedule, 9-5 hours
Location: New York
Responsibilities
Assess clients' substance abuse, health, mental health, and other care needs; recommend internal and external referrals
Provide individual and group counseling
Prepare psycho-socials
Facilitate psycho-education groups in relapse prevention, health literacy, and navigating mental health and substance abuse systems, etc. for residents their family and/or support networks
Provide Comprehensive Case Management services
Assist clients in improving and maintaining self-care, interpersonal, and vocational skills
Assist clients in achieving housing stability
Assist in the management of discharge planning; engage and support clients 90 days post discharge;
Manage client documentation, provide program narrative and/or statistical reports as needed; Participate in ongoing QI and program evaluation activities
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
Ability to learn and provide evidence-based treatment practices
Knowledge of treatment interventions and wellness management practices for persons chronic health conditions, as well as serious mental illness and substance abuse disorders
Knowledge about administering assessment tools and/or the ability to learn
Ability to deliver individual, group, and family counseling services
Ability to efficiently and accurately record services provided, including in AWARDS, CARES, or similar systems
Ability to create accurate and descriptive reports
Strong interpersonal skills

JOB SPECIFICATIONS
Education and Experience Requirement(s):
Masters Degree in Social Work (MSW) required; New York State Licensed Masters in Social Work preferred
Two years experience with homeless population
Two years' experience with persons with chronic health and/or mental health conditions and/or substance abuse disorders
Bilingual in Spanish preferred
Physical Demands:
Non-physical.
Working conditions:
Operates in a professional office environment with frequent travel to local offices to meet the training schedule needs of 24/7 residential social service environment.
Routinely uses standard office equipment such as computers, phones, copiers, filing cabinets and fax machines.
Business hours are Monday to Friday, 9:00 a.m. to 5:00 p.m. Because program operates 24 hours a day, 7 days a week, availability to work other hours and respond to time sensitive matters is required.
SAFETY RESPONSIBILITY

Immediately report all unsafe practices, conditions, and/or equipment;
Immediately report any accident or injury

Conduct oneself in a manner which does not interrupt or interfere with and is conducive to the harmonious operation of the company's business.
