Responsibilities
Acts as Broker on behalf of assigned self-directing individuals. Works closely with the self-directing individual and their circle of support to develop a budget for services that fits their unique needs and goals. Maintains a person-centered, personalized approach to the service planning process. Assists self-directing individuals with tracking budget expenditures. Works closely with the Fiscal Intermediary and Medicaid Service Coordinator to ensure open communication and high quality services. Complies with OPWDD and agency policies and procedures regarding compliance and Medicaid billing for all programs as assigned.
Qualifications
Requires Associate's degree in Human Services field, Bachelor's degree preferred. Experience with Microsoft Excel and/or accounting background preferred. Must have a valid New York State Driver's License, a reliable vehicle, meet Lifetime Assistance vehicle operator requirements and agency standards to transport service recipients.

Commensurate with experience.
