Our programs include comprehensive counseling services for all ages, basic emergency assistance including operating 8 food pantries, mental health services, chemical dependency treatment, educational and job readiness services, services to older adults, marriage counseling, immigration and refugee assistance and a variety of programs that address prevention and treatment of a number of family issues.40 hour work week14+ paid holidays for FT25 PTO days a yearComprehensive health, dental & vision coverageFlexible Spending Account optionHealth Savings Account when elect a high deductible health planFree Employee Assistance Program (EAP)403B Retirement Plan - (may join after first pay period) with employer contribution after six monthsJOB SUMMARY:Site Facilitators serve as the point of entry for school-based community collaboration that supports student achievement by addressing nonacademic barriers to academic success and ensuring the outcomes of Closing the Gap (CTG) collaborations are being met.ESSENTIAL FUNCTIONS:Exemplify Catholic Charities' mission, vision, guiding principles and policies and procedures Promote adherence to high ethical standards, consistent with the Catholic Charities' Corporate Compliance Program, Code of Ethics, legal mandates & labor laws, NYS Justice Center, NYS, OMH, managing contractual regulations and other accreditation standards (e.g. COA, BBB). In conjunction with school leadership and support teams, facilitates school-wide outcomes of improved attendance, improved discipline, community agency family support, mental health services, social-emotional supports, parent involvement and developmental assets. Creates single point of entry referral system for students and families needing services and supports. Serves as liaison between service providers, school building staff and administration, including educating and informing school staff regarding the services and programs available and how to refer students and families. Works closely with school staff to identify at-risk students & families in need of support and connects to meaningful interventions through the use of building data. Supports school-wide initiatives & special projects for at-risk students Coordinator of mental health and crisis concerns: - Assist students/families with crisis or needs support- Implement crisis intervention, de-escalation and/or coping skills- Collaborate with providers to support students with and without provider services and/or supplement social emotional learning- Facilitate student social emotional learning, and attendance groups as appropriate Develops and coordinates relationships with potential partner providers and community resources; Ensures provider services are aligned with school goals and culture; Ensures providers are meeting targeted outcomes Generates and disseminates referrals for services. Serves as an active member of the school's management team and/or other teams as appropriate. Makes changes in service provision as necessary to meet school needs. Responsible for service tracking and reporting. Represents Catholic Charities in various school initiatives and on various school teams. Leverages Catholic Charities services (i.e. programs serving youth and families; PR/Communications- social media and webpage promotion) to foster intra-agency collaboration, as well as to strengthen external collaborations. Participates in various Catholic Charities and school community outreach events with some non-traditional hours required. Allows for communication with families during nontraditional times as appropriate and fitting Engages in job related travel Performs other duties as assigned.EDUCATION: Bachelor's Degree in Social Work, Human Services, Education, Counseling or a related degree; or equivalent combination of education and experience considered; Master's Degree preferredEXPERIENCE, KNOWLEDGE, SKILLS AND ABILITIES: The ideal candidate must possess school and community experience, expert relationship management skills, with at least 1 year experience working with educational systems. Proficient in Microsoft Office products (Word, Excel, etc.) database management.SPECIAL REQUIREMENTS: Requires travel - must possess valid NYS driver license and agency-required level of auto insurance coverage and be able to travel as needed within the eight county WNY region.
