Position Summary
The Nurse Supervisor is responsible for supervising the delivery of care to residents within the nursing community. Performs a variety of duties to ensure quality nursing care is provided to residents to maintain or attain the highest practical level of functioning; implementing specific procedures and programs; participating with the charge nurses and the Director of Nursing in establishing specific goals; determining work procedures and expediting work flow; ensuring compliance with all operating policies and procedures and Pennsylvania Department of Health regulations and Federal Health Care Administration regulations; providing periodic reports; responsible for the management of the operations of assigned units.
Perform other duties as required.
Essential Duties and Functions
Resident Care, Advocacy & Safety
• Directs and supervises activities of the neighborhoods for the provision of nursing care services
• Makes rounds in the entire health center at a minimum of twice per shift; monitoring for resident safety, and infection prevention and assessing resident care
• Evaluates outcomes of nursing care and functions under scope of responsibility on a regular basis to improve quality and plan better utilization of staff time and activities
• Reviews resident care being delivered by nursing personnel and monitors care outcomes daily according to the plan of care
Audits & Quality Assurance
• Prepares narrative and statistical reports documenting unit activities as required/requested by Nursing Administration; monitors completion of nursing documentation for the shift.
• Conducts audits as assigned
Compliance, Reporting & Policies
• Supports the client mission, objectives and administrative policies
• Demonstrates knowledge of nursing practice standards in directing resident care activities
• Assist in the development and implementation of nursing department policies and programs
• Ensuring adequate staff coverage to meet proper ratios, PPD and acuity of residents according to daily census
• Conducts resident incident investigations; communicates verbally and in writing the results of the investigations, outcomes, and recommendations for the plan of action
Clinical Processes
• Performs Registered Nurse duties approved under their scope and practice
• Analyze and audit information to develop appropriate interventions to prevent decline. Ensure changes in condition are recorded and followed up on appropriately
• Confer with residents and their families; make referrals to other departments when appropriate
• Communicate status of residents to oncoming personnel; communicate with physicians; transcribe, clarify, and implement physicians' orders as needed
• Audit medical records, including assessments, changes in condition, incidents, etc. and document as required
• Assesses, assists and manages all emergency occurrences during the shift
• Resolves problems through assessment, review of potential solution; involves others in problem solving process
• Assess, assist and manage all emergency occurrences
• Coordinates incident investigations, gathers data, assesses information, communicates in writing the results of the investigation, outcomes and makes recommendations for the plan of action
• Complete and audit resident admission including the development of care plan; attend and participate in the Plan of Care meetings as needed.
• Respond to emergencies; assess the condition of the resident; and formulates a correct plan of action
Qualifications:
Prior experience of six (6) months providing care in a SNF or hospital preferred
Graduate of accredited school of nursing
Current, active RN license; current CPR certification
Language Skills:
Ability to listen to and understand information and ideas presented through spoken word and sentences, and the ability to communicate information and ideas in speaking so others will understand.
