- Pay:
- $35 - $42/hour based on date of licensure
- PLUS $3/HR in PM shift differential
- BONUS - Paid 40 hours for working 36!

Responsibilities:
Responsibilities:
- Able and willing to perform all duties encompassed in the Resident Assistant/STNA Job Description and the Staff Nurse Job Description whenever needed.
- Works from a team approach to guide staff in meeting residents’/patients’ needs.
- Establishes an excellent rapport with residents/patients and their families.
- Coordinates staff assignments, and maintains appropriate staffing ratios.
- Immediately follows up with Staffing Coordinator or on-call if unexpected staffing concerns arise during the shift.
- Delegates duties to staff members within each individual team member's scope of practice, training, and competency level.
- Follows up on delegated tasks to verify the task was completed.
- Reviews PointClickCare 24 hour summary every morning and listens to report at the beginning of every shift to be aware of all issues in the building.
- Attends Morning Report, Orders Reconciliation Meeting, Interdisciplinary Team Meeting (IDT), Fall/Skin/Quality Meeting, CASPER Meetings, Bedside Rounds, and all other meetings requested by the Directors of Nursing.
- Records meaningful report at the end of the shift for the oncoming Nurse Manager. The report shall include all areas critical for follow up, including changes of condition, abnormal labs, incidents, etc.
- Responds to urgent and emergency needs immediately.
- Directs injured staff members in incident report completion and appropriate workers’ compensation procedure as appropriate.
- Assesses and reports physical, mental and emotional changes in resident’s/patient’s condition as well as all accidents/injuries to the attending physician (or designated on-call practitioner) and to the resident's/patient’s responsible party.
- Ensures that physician's orders are carried out correctly, in a timely manner, and according to state and federal regulations and facility policies and procedures.
- Transcribes telephone physicians' orders, as well as new admission and readmission orders. Calls the attending physician (or on-call practitioner) to review all orders via telephone as soon as transcribed.
- Collaborates with team to complete admission paperwork and follows up with staff to make sure everything is completed.
- Delegates the coordination of appointments with physicians' offices/clinics with Colonial transportation and families to the unit clerk and follows up to ensure timely arrangements are completed.
- Follows up with physician regarding lab and diagnostic test results.
- Follows up with physician regarding pharmacy recommendations.
- Follows up on resident/patient needs identified from Morning Report, IDT Meeting, Bedside Rounds, or Wound Rounds.
- Assigns weights and follows up with staff members to ensure timely completion with request for re-weights as appropriate.
- Provides timely communication with culinary, dietitian, dietetic technician, and/or therapy staff as needed for weight or diet concerns.
- Prepares for physician rounds on days when Physicians, Physician’s Assistants, or Advanced Practice RN’s will round.
- Reminds and assists Physicians, Physician’s Assistants, Advanced Practice RN’s or Pharmacists to log hours for Payroll-Based Journal (PBJ).
- Completes or delegates the completion of assessments, SBAR, UTI SBAR, QI Tool for Review of Acute Care Transfers, STOP AND WATCH Tools, Fall Investigations, and other assessments or tools as appropriate.
- Monitors Controlled Drug Logs (narcotic sheets) every shift and maintains a list of needed prescriptions for those controlled drugs that will need refilled on the next Physician, Physician’s Assistant, or Advanced Practice RN’s rounds.
- Administers IV medications, IV fluids, and IV line maintenance and needed according to physician's orders.
- Maintains supervision of care provided by Resident Assistants and Staff Nurses.
- Maintains staffing ratios according to unit census.
- Reviews all incidents for the past 24 hours and follows up with physician, responsible party, nursing and/or therapy as appropriate.
- Follows up on all hospital transfers and completes and distributes QI Tool for Acute Care Transfers within 24 hours of transfer from the facility to a hospital. These are uploaded to the Nurse Manager email distribution list, which includes the QI Nurse.
- Initiates and completes Baseline Care Plan within 48 hours of admission to facility.
- Provides ongoing support to the Staff Nurses and Resident Assistant/STNA’s.
- Completes performance appraisals as scheduled.
- Knows the Butler County Coroner’s Guidelines for Reportable Deaths and acts accordingly in the case of deaths that meet the criteria.
- Attends Care Conferences as needed.
- Provides ongoing feedback to team members regarding their performance.
- Provides prompt and accurate response to staff, resident, and family inquiries for information while always honoring confidentiality.
- Reviews team member's documentation and provides guidance, support, and education where needed.
- Initiates and follows through with disciplinary action when necessary, including completion and presentation of proper forms.
- Promotes adherence to resident Care Plans to guide and direct care.
- Enforces infection control policies and procedures.
- Enforces proper medication administration and handling procedures according to state and federal regulations.
- Performs audits to ensure ongoing compliance with policies and procedures as assigned by the director of nursing.
- Maintains professional knowledge base.
- Documents pertinent information using established policies/guidelines.
- Maintains continuity of care.
- Maintains confidentiality.
- Communicates job/resident related questions/concerns, and suggestions through the established chain of command.
- Provides a safe and clean environment.
- Completes and updates daily census as changes are made to census and staffing every shift. Copies are saved to G drive file every shift.
- All other duties as assigned and/or appropriate to the position.
Requirements:
- Education - Graduate from an Ohio Board of Nursing approved nursing program or school of nursing.
- Licenses - Must have an active RN license in the state of Ohio.
