We are looking for a compassionate, caring, Memory Care Coordinator to join our family of Caregivers. As the Memory Care Coordinator, you will have the ability to be surrounded by amazing Residents daily and will develop a collaborative workplace with attention to quality care for our Residents.
The Memory Care Coordinator is responsible to assist the DON with the daily functioning of health care needs of Residents as well as supervision of the memory care department. The Memory Care Coordinator is responsible for all residents in memory care and the supervision of memory care services to residents. They are responsible for directly training staff as well as supervising all memory care staff. The Memory Care Coordinator will work with doctors, residents, families, home health staff, and hospice companies ensuring resident's needs are being met.

Duties/Responsibilities:
• Assists with the supervision of nursing staff and the medication delivery system
• Assists with training new caregivers along with training for existing caregivers
• Assists with planning and overseeing admission, nursing, and patient care process
• Adheres to practices in accordance with the regulations of the state
• Assists in overseeing development, oversight, and maintaining Quality Assurance Program for nursing staff
• Orders, Stores, and assists with medication according to physician orders including keeping up with expiring medications and their refills
• Assists with Resident care, meals, move-outs, and move-ins as needed
• Observes and communicate any changes in the resident's condition
• Completes and reviews incident reports as needed
• Maintain current Resident records for each Resident including but not limited to; obtaining current physician orders, clarifying orders, and alert monthly charting
• Answering, and responding to call lights, and door alarms as needed
• Knows and follows Emergency Procedures, Elopement, and all other procedures
• Attends and completes all assigned training for DSS regulations
• Keeps resident information confidential; respecting Resident rights; respecting privacy
• Keeps physician order's and care plans up to date
• Ensure dry checks are done
• Ensuring treatments are completed
• Provide ongoing observation of the residents
• Mandated Adult Abuse reporter
• Must report safety issues
• Must be able to work weekends as needed and rotate on call schedule
• Must always be available to take phone calls
Job Qualifications:
• Must be certified as a Registered Nurse with Memory Care Experience
• Must have good organizational skills and supervisory skills
• Willing to praise and discipline team members as appropriate
• Must have an acceptable criminal record check
• Documented TB Screening
• Must have or complete CPR/1st Aide within 60 days of employment
• Must be able to perform Resident care with awareness of dignity and individuality, with understanding and concern
• Must perform unpleasant tasks with grace; works cooperatively with supervisors and display a willingness to assist co-workers
• Must abide by established policies and procedures of the community
