Now Hiring: RN MDS Coordinator (Full-Time)

Responsibilities: 

• Coordinate and manage the MDS assessment process, including scheduling, data collection, and submission.
• Ensure accurate and timely completion of all MDS assessments in accordance with regulatory requirements.
• Conduct regular audits of MDS documentation to ensure accuracy and compliance.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans based on MDS assessments.
• Provide training and support to nursing staff on MDS procedures and documentation.
• Monitor and analyze quality indicators and implement improvement initiatives as needed.
• Maintain up-to-date knowledge of MDS regulations and best practices.

Qualifications:

• High school diploma
• RN MDS experience necessary
• Leadership qualities
• LTC/SNF experience
• Serious applicants
