LPN/RN Clinical Evaluator
What You'll Do:
As the Clinical Evaluator, you will be responsible to review hospital referrals for our nursing home, complete an assessment of the resident’s condition, have completed the Universal Financial Application, and gather the necessary clinical information from the hospital that is required for the care of the patient.
Primary location is the Saratoga area.
Key Responsibilities:
- Perform comprehensive assessments of physical, mental, and emotional health to determine appropriate sub-accute care and rehabiliation.
- Develop and maintain positive relationships with area hospitals and community agencies
- Establish and maintain relationships with social workers, case managers, physicians and other contacts in the healthcare and payor community
- Educate residents and families about facility services, procedures, and care expectations
- Obtain insurance authorizations when needed.

Qualifications of a Clinical Evaluator include:
- RN or LPN license required.
- Strong clinical skills, critical thinking abilities, excellent communication skills and ability to be self-directed are essential.
