Job Title: RN/LPN MDS Coordinator

Location: Amesbury, MA

Salary: $46.18 - $48.12 per hour

We are seeking a dedicated RN/LPN MDS Coordinator to join our team. Responsibilities include:

Conducting comprehensive assessments for residents

Coordinating the MDS process and ensuring compliance with regulations

Collaborating with interdisciplinary teams to develop care plans

Training staff on MDS-related policies and procedures

Monitoring resident outcomes and quality measures

Qualifications: Current RN or LPN license in MA, experience in long-term care, and knowledge of MDS processes. Strong communication and organizational skills are essential.
