About The Position
**Job Title:** Registered Nurse (RN) - Home Care **Job Specialty:** Home Care **Job Duration:** 13 weeks **Shift:** 8-hour days, Monday to Friday (8:00 am - 4:30 pm), with on-call weeknights and weekends on a rotational basis **Guaranteed Hours:** 40 hours per week **Experience:** At least 1 year as a Home Health RN **License:** Current State of Illinois RN license **Certifications:** Basic Life Support (BLS) **Must-Have:** - Patient assessment skills - Medication management - Wound care expertise - IV competency (PICC, peripheral) - Strong communication and patient education skills **Job Description:** - Conduct patient education and ensure comprehensive patient assessments, including vital signs and head-to-toe evaluations. - Perform skin and wound assessments, including dressing changes and wound care procedures using specialized equipment like Wound Vac. - Manage and care for PICC lines, and perform lab draws. - Change and care for foley and suprapubic catheters and manage ostomies. - Conduct medication reconciliation and provide patient and family education. - Document patient interactions and interventions using Epic software. - Collaborate with an interdisciplinary care team, including healthcare providers, therapy staff, social workers, wound care centers, and infusion teams, to offer coordinated patient care. - Utilize department equipment such as laptops, portable printers, and cell phones for effective patient management and communication. - Clinicians are expected to see an average of 5 patients per day and complete documentation appropriately. **Additional Information:** - Professional attire, such as scrubs, is expected. - Orientation includes staff development, wound care training, IV classes, online modules via Canvas, and general training lasting approximately 5-7 days. - Call-off policy allows for the equivalent of 12 hours every 13 weeks without penalties or need for compensation. - Local candidates within a 50-mile radius are welcome to apply.
