In this inspiring role, you hold the power to perform comprehensive assessments, collaborating seamlessly with our multi-disciplinary team of experts, including PT, OT, ST, MSW, and paraprofessionals. Together, we will work passionately to achieve the heartfelt outcomes and goals of our patients, leading them on a journey of healing and hope.
Supervise your caseload, nurturing the growth and development of LPNs and Home Health Aides, as we cultivate a team of caregivers. Qualifications
- Active RN License in state of service
- One year RN nursing experience or three years LPN/LVN nursing experience providing direct patient care in a home care setting.
- Must obtain and maintain CPR Certification

Pay Range
$32.98 - $47.82 /hour
