RN Clinical Manager - Health Care Associates and Community Care Givers

Community Care Givers is a family run home care agency seeking a FT RN Clinical Manager M-F, 32-40 hours with some flexibility to set your schedule. Experience in home health is not necessary as we will train the ideal candidate.

Job Duties:

Candidate would provide oversight of all patient care services and personnel including but not limited to:

·        Making patient and personnel assignments

·        Coordinating patient care

·        Coordinating referrals

·        Assuring patient needs are continually assessed

·        Assuring the development, implementation, and updates of the individualized plan of care.

Qualifications:

·        Current State of Michigan Registered Nurse License

·        Demonstrated ability to supervise and direct professional administrative personnel is a plus.

·        Excellent observation skills

·        Verbal and written communication skills.
