Job DescriptionPosition Summary:
The Clinical Manager plays a crucial role in coordinating and managing patient care. This position ensures that all care and services are delivered appropriately and is responsible for the supervision of clinical personnel.
- Case Management: Receive and review case referrals, assign appropriate clinicians, and evaluate services provided to ensure quality home care. Guide and support clinicians in developing care plans and achieving therapeutic goals.
- Clinical Oversight: Review patient diagnoses, medications, and procedures. Lead case conferences, assess clinician performance, and provide ongoing mentorship through field visits and 1:1 meetings.
- Team Leadership: Uphold and enforce Agency policies and procedures, promoting alignment with the Agency's Mission, Vision, and Values. Participate in the hiring, orientation, and assessment of new personnel.
- Compliance & Standards: Ensure adherence to professional standards, maintain compliance with SOPs, and consistently promote the use of patient portals, Telehealth, and other relevant tools.
- Community Engagement: Participate in public relations and community activities to enhance the Organization's role within the healthcare system.
- Additional Duties: Perform other tasks as delegated by the Director of Clinical Services.
- Registered nurse with current licensure to practice professional nursing in the state.
- Graduate of the National League for Nursing or an accredited school of nursing. BSN degree is preferred.
- Two years of management or supervisory experience in a health care setting, preferably home care.
- Complies with accepted professional standards and practice.
- Complies and maintains current CPR certification.
- Demonstrates excellent observation, organization, and verbal and written communication skills.
- Proficient in Computers, Microsoft, EMR, and Technology.
- Knowledge of governmental regulations and ACHC standards.
- This position requires that the employee is willing and able to meet the physical requirements as necessary.
