RN CLINICAL CALLS SPECIALIST – Position Summary
The RN Critical Calls Specialist will manage clinical calls for hospice patients and primary care givers to manage symptoms, durable medical equipment needs and medication refills. RN will collaborate with the clinical care team to provide seamless and immediate responses to patient needs within the plan of care. Additional administrative duties to coincide with patient care and assist the clinical manager team for staff support.
RN CLINICAL CALLS SPECIALIST – Essential Duties & Responsibilities
· Answers telephone calls and appropriately provides triage for telephone calls from patients, caregivers, facilities, pharmacies, and any other clinical resource that is related to patient care. Provides rapid assessment via telephone as indicated. Performs follow-up phone calls as needed.
· Educates patients and caregivers about methods of symptom management.
· Utilizes the EMR to access patient records remotely and to review patient’s condition, relatable signs and symptoms and evidence-based treatments.
· Accurately documents symptoms/complaints, nursing assessment, advice provided and patient/caller response in the patient’s electronic medical record. Follows policies, procedures, and protocols to ensure consistency and continuity of patient care.
· Assists with ordering of supplies, medications, and DMEs as needed.
· Notifies RN Case Managers of the calls, any symptom intervention initialized and patient visit needs in a timely manner.
· Accesses and uploads clinical documents from hospitals EMR, facilities, outside providers including, but not limited to, discharge summaries, lab results, H&P for current patients and upload to EMR. Provides information to clinical staff as requested regarding patient documentation.
· Communicates with appropriate clinical staff and members of the interdisciplinary team
· Assists with calling high acuity patients as part of emergency preparedness or other urgent situations to ensure patient safety, continuity of care, and address needs that may arise as requested by the Vice President of Clinical Operations.
· Plans nursing interventions in collaboration with the interdisciplinary team to resolve current problems and prevent potential problems.
· Acts as a patient advocate.
· Delivers care within the scope of nursing practice and in accordance with accepted standards of practice.
· Provides emotional support to patients and families in a manner that fosters independence, affirms existing and new strengths, and promotes and develops strengths.
· Provides continuity of service across care setting through effective information management, including both verbal and written communication.
· Respects the autonomy and right to confidentiality of all patients and families.
· Seeks opportunities for professional growth, including demonstrating the ability to receive constructive performance feedback.
· Acts independently and responsibly to perform job duties on a consistent basis.
· Is sensitive to the stressors experienced by colleagues in working with Hospice patients and families.
· Carries out the mission and values of Blue Ridge Hospice.
· Must be a licensed driver with an automobile that is insured in accordance to state or organization requirements and is in good working order.
· Attends and participates in department and nursing meetings.
· Other duties as assigned to assist the clinical management team
RN CLINICAL CALLS SPECIALIST – Qualifications/Experience
· Licensed Registered Nurse with current Virginia licensing per state board of nursing.
- Certification in Hospice and Palliative Care nursing preferred.
· Bachelor’s degree in nursing preferred.
· Two years hospice experience preferred.
· Excellent oral and written skills.
· Proficient computer skills and electronic medical records experience preferred.
