About The Position
Position Title: RN - Supervisor MDS Coordinator
Facility Address: Kansas City, MO
Full-time
Shift times and shift length: 08:30am-5:00pm
Base Hourly Pay/Salary Range: Based on experience - $34.70+/hour

Job Description/General Duties:
- Participate in discharge planning
- Participate in all meetings
- Based on staffing needs, available to provide supervision and direct care for LTC residents.
- Generate- and provide a schedule of all MDS assessments, update new admits as needed
- Coordinate the completion of the MDS/CAAs/Care Plan
- Conduct a comprehensive, accurate, standardized assessment of each Resident upon admission
Minimum Requirements:
- Registered Nursing degree
- Current license to practice as a RN in the State of Missouri. AHA BLS
- Three years of direct care experience, one year long term care experience
- Possess current knowledge of CMS regulations and guidelines, MDS requirements and procedures, and PDPM procedures.
- Expertise using computer software related to production of Care Plans and MDS's
- PCC or MDS experience
