A leading not-for-profit health system in Oklahoma City is seeking a full-time RN Case Manager for its Trauma Unit, working from 8 a.m. to 4:30 p.m. The role involves managing clinical and financial outcomes for a patient caseload, coordinating care, and facilitating timely discharges. Responsibilities include:

• Conducting comprehensive patient assessments.
• Coordinating transfers and discharge plans.
• Communicating care plans to patients and healthcare teams.
• Ensuring appropriate resource utilization.
• Maintaining knowledge of regulations and community resources.

Applicants should have 5 years of clinical experience, a current RN license in Oklahoma, and strong communication skills. A BSN and Case Management Certification are preferred.
